Change the behavior of how :visible and :hidden work. :hidden is when an element is display none, a parent element is display none, or the element has a width of 0. :visible is when the element is not display none and all of its ancesotrs are not display none and its width is larger than 0. After discussing it with a number of users I've decided to revert the change to [name!=value]. It is now equal to :not([attr!=value]). Attempting to switch it to [attr]:not([attr!=value]) produced some really non-obvious results and confused users. Fixes jQuery bug #3884.

Fixed tabindex normalization so that elements that natively support tabbing, but don't have a tabindex explicitly set return 0 instead of undefined.
Removed jQuery.support.tabindex since we're only normalizing non-XML right now and all browsers support tabIndex for HTML documents.
